Quantitative effects of head rotation angle on apnea hypopnea index in positional obstructive sleep apnea – a preliminary case series

Abstract Objectives Quantify the effects of head rotation and incline on obstructive sleep apnea (OSA) severity. Design Single-arm, intervention study. Setting Pulmonary specialty clinic. Case presentation Ten adults diagnosed with positional OSA ranging from 32 to 64 years age 6 females, 4 males reporting persistent daytime sleepiness health issues consistent use CPAP. Intervention Standard polysomnography a angle sensor attached forehead coaching fall asleep at various angles torso in supine non-supine positions. Measurements severity was scored according American Academy Sleep Medicine guidelines. Apnea hypopnea index (AHI) peripheral capillary oxygen (SpO 2 ) saturation were measured during each epoch unique rotation, incline, position. Results Two participants (1 no apneas 1 central apnea) excluded. Among remaining 8 participants, average reduction peak AHI 66% (range 18–88%) ≤ 20° above horizon compared > horizon. The values significantly lower than (20.0 vs 45.3, P = 0.002). Minimum SpO higher for (mean: 90.6% 84.3%, 0.03). In position, (7.1 52.1, < 0.001). not statistically significant (22.3 38.4, 0.09).
